What is a head spa?

A head spa is a professional treatment for the scalp, the skin your hair grows from. A typical session checks the scalp first, cleans it more thoroughly than a home wash can, spends a long stretch massaging the head, neck and shoulders, and finishes with warm flowing water and steam. Put simply, the answer to what is a head spa is scalp care with a long massage built in.

The version most people picture, with a scalp analysis, layered cleansing, steam and a waterfall rinse from an arch above the basin, is the format usually called a Japanese-style head spa. Most menus are assembled from the same 4 parts:

  • Scalp analysis. A close look at oil, buildup, flaking and redness before anything is applied, so the products suit your scalp. Some spas repeat it at the end so you can compare.
  • Deep cleansing. Cleansers worked into the scalp section by section to lift the oil, dry shampoo and styling product a quick wash leaves behind.
  • Massage. Usually the longest part, covering the scalp and often the neck, shoulders and arms.
  • Water and steam. Warm water flowing over the scalp from an arch or ring above the basin, often with steam as well.

How is a head spa different from a salon shampoo?

A salon shampoo cleans the hair in a few minutes before a cut or color; this treatment spends most of an hour on the scalp underneath. That focus lines up with dermatologists' own advice. The American Academy of Dermatology (AAD) tells people to apply shampoo to the scalp instead of the entire length of the hair.

The massage is the other big difference: at the shampoo bowl it lasts a minute or two, if it happens at all, while here it is the main event and usually extends into the neck and shoulders. Massage is common and generally low risk: national survey data cited by NCCIH show 10.9 percent of U.S. adults used massage therapy in 2022, up from 4.8 percent in 2002, and NCCIH says the risk of harmful effects from massage appears to be low.

Is a head spa the same as a dermatologist's scalp treatment?

No. A dermatologist diagnoses and treats scalp and hair conditions; a head spa cleans and relaxes a healthy scalp. When someone comes in with hair loss, the AAD says dermatologists ask when it started, look closely at the scalp and nails, and test the hair with a gentle pull. They may add blood work or a scalp biopsy if they suspect a disease, deficiency, hormonal imbalance or infection. Treatment depends on the diagnosis, and the AAD states plainly that no one hair loss treatment works for everyone.

A head spa's scalp analysis is a cosmetic check of oil and buildup. It cannot tell thinning from shedding or spot an infection, and nobody at a spa should claim it can. Use each for its own job: the dermatologist for a diagnosis, the spa for a clean, comfortable scalp and an hour of massage.

What happens during a head spa at O2?

Package D, the Classic Scalp Treatment, is O2's base session and runs 60 minutes. Most of it happens lying on a treatment bed with your head resting over a built-in basin, like the one in the room pictured at the top of this guide, and a folded towel over your eyes while the water runs. After the first few minutes there is nothing for you to do except say if the water feels too warm or the pressure too firm.

The booking page lists 8 parts to the session, and every longer package keeps them. Here is each part in plain terms; if you want to know the order on the day or how a step feels, ask your therapist before you lie back.

  • Pre and post scalp analysis comparison. Your scalp is checked at the start and again at the end, so you can see the before and after side by side.
  • Aromatherapy oil massage. Massage with a scented oil.
  • Head, neck, shoulder and arms massage. The massage moves past the scalp into the neck, shoulders and arms.
  • Deep scalp cleansing. A thorough clean of oil and product buildup at the roots.
  • Waterfall hydrotherapy and meridian therapy. Warm water runs from a gold arch over the basin and through your hair, the step most photos show. Meridian is a term from traditional Chinese medicine.
  • Nano steam therapy. A fine steam over the scalp.
  • Scalp serum spray. A serum misted onto the scalp.
  • High frequency ozone comb therapy. A high frequency comb device passed through the hair.

What do Packages C, B and A add?

Each longer package keeps Package D and adds more massage and treatment time. Package C runs 75 minutes and adds a step the booking page calls "hair growth / strengthening nourishment", a hand, arms, neck, shoulder and head massage, a phototherapy meridian comb treatment, and high frequency ozone comb therapy.

Package B and Package A both run 90 minutes and build on Package C. Package B adds a hand, arms, neck, shoulder and head massage plus one add-on of your choice: a mini facial, a hand treatment or a neck treatment. Package A adds a back, arms, legs, neck, shoulder and head massage with hot stone therapy, which makes it the closest thing on the menu to a full massage with the scalp treatment attached.

Every package has two prices on the booking page, a regular rate and a member rate at half the regular price. The booking page doesn't explain how membership works, so ask the front desk before you book.

Who is a head spa good for?

A head spa suits people who want a thorough scalp clean and a long, quiet massage in the same appointment. That includes people whose scalp feels oily or coated from dry shampoo and styling products, people who carry tension in the neck and shoulders, and anyone curious about the format who wants to start with the base session before trying a longer one.

Scalp condition matters more than hair type, because the work happens at the roots. Mention recent color, a sensitive or itchy scalp, or a fragrance you dislike when you book, so the therapist can adjust products and pressure.

Some situations call for waiting. Put off a booking if your scalp has open cuts, a fresh sunburn, an infection or a recent procedure your doctor hasn't cleared, and check with your doctor first if you have a condition that affects your skin or circulation.

What can't a head spa do?

A head spa cannot diagnose or treat hair loss, dandruff that won't clear, or any scalp condition. Some shedding is expected: the AAD says it's normal to shed between 50 and 100 hairs a day. Hair loss, by the AAD's definition, happens when something stops hair from growing, and the sooner treatment begins, the better the outlook. If your hair is noticeably thinner or coming out in large amounts, see a board-certified dermatologist before you book a spa.

Flaking deserves the same care. Mayo Clinic lists several causes of dandruff, including irritated oily skin, dry skin, sensitivity to hair care products and a fungus called malassezia that lives on the scalps of most adults, and notes that dandruff can get worse with stress and in cold, dry weather. A deep cleanse may lift visible flakes for a while, but the AAD's first step is a dandruff shampoo with an active ingredient such as zinc pyrithione, salicylic acid, selenium sulfide or ketoconazole, and some need to sit on the scalp for 5 to 10 minutes before rinsing. If regular use of a dandruff shampoo doesn't help, Mayo Clinic advises seeing your healthcare professional.

How often do people get a head spa?

There is no medical schedule for this, so frequency comes down to your scalp, your budget and how much you enjoy it. A practical way to decide: book one session, notice how many days your scalp feels clean and light afterward, and book the next one when that feeling has gone. Some people come monthly; others save it for a stressful stretch or a special occasion.

Your daily routine does most of the work between visits. The AAD's advice is to match shampoo frequency to your hair: people with straight hair and an oily scalp may shampoo daily, while dry, textured or curly hair can go as long as every 2 to 3 weeks.

How should you prepare for your first head spa?

Preparing takes very little: book online or call O2 at (626) 857-6608, and leave enough time for the full session, from 60 to 90 minutes depending on the package.   • Go light on styling products that day. Many spas prefer to see the scalp as it normally is; ask when you book whether O2 would like you to arrive unwashed.

  • Say what you want from the massage. Pressure, water temperature and how much you feel like talking are all worth mentioning, during the session as well as before it.
  • Leave a quiet gap afterward. A long scalp massage can leave you drowsy, so avoid booking one right before something that needs full focus.
